What is an FPM?
A collaborative team discussion, held at critical decision points during a child welfare case, among families, family supports, community partners, the local Department of Social Services and children ages 9 and up when appropriate.
When do FPMs occur?
- When services and supports are needed to keep a child safe in their own home
- When a child is no longer able to remain safely in their home
- When a foster child experiences a disruption in their placement
- Prior to a change in goal for a foster child when updating the foster care service plan
- At key decision points and when there is a need for collaborative planning
Who Attends an FPM?
- Family members and other close supports such as friends or neighbors
- DSS caseworkers and supervisors
- Counselors, teachers, service providers
- Coaches, pastors, youth group leaders
- Guardian ad Litem (GAL), Court Appointed Special Advocate (CASA)
What is the goal of the FPM?
- Empower the family and plan for the best possible decisions for them and their child
- Reach an informed consensus on a plan for the family and child that addresses placement, services, and permanency decisions
- Safety and well-being of the child is the foundation of the decision
What happens in the FPM?
- A trained facilitator reviews group guidelines to ensure that everyone has an opportunity to speak and is treated with respect
- The family and/or Family Services Specialist identifies the reason for the meeting
- The team identifies the child and family’s strengths and needs
- The team shares ideas on how to best meet the child and family’s needs
- The group will develop plans for the child and family with the goal of consensus about the child’s placement and services
