On July 1, 2016, Henrico Area CSB began serving as the single point of entry for individuals with Developmental Disabilities (DD) for Henrico, New Kent, and Charles City counties. If an individual or family member requests placement on the DD Waiver Waitlist, the I/DD Intake/Eligibility team will assess diagnostic and waitlist eligibility.
Individuals who have a diagnosis of developmental disability (DD) meet diagnostic criteria that includes: A documented severe, chronic disability that is attributable to a mental or physical impairment or a combination of mental and physical impairments, other than a sole diagnosis of mental illness and is manifested before the individual reaches 22 years of age.
- Is likely to continue indefinitely.
- Results in substantial functional limitations in three or more of the following areas of major life activity to include:
- Self-care
- Receptive and expressive language
- Learning
- Mobility
- Self-direction
- Capacity for independent living
DD Waiver Case Management/Support Coordination Services are available to individuals who have a diagnosis of a Developmental Disability and meet diagnostic criteria, financial criteria and have a developmental disability waiver. The individual would need to have access to one of three DD waivers: Community Living Waiver, Family & Individual Support Waiver or Building Independence Waiver. Case Management services can be provided by a Henrico Area Mental Health and Developmental Services Case Manager/Support Coordinator (CM/SC) or the Regional Contracted Private Provider. The CM/SC role is to link the individual to supports and services that are needed for the individual to integrate into the community. The CM/SC would complete an assessment to identify those needs. The CM/SC would provide monitoring of supports and services to ensure individual satisfaction and health/safety.
DD Waitlist Monitoring
If placed on the waitlist, the individual will be assigned for DD waitlist monitoring. This includes yearly follow up, update to any status changes and advocate for waitlist individuals to obtain a waiver slot. Waitlist Monitoring does not include ongoing/active level Case Management/Support Coordination Services.
