Application for an amusement device permit (see documents).
Fees
The following permit fees are for each ride:
- Small mechanical or inflatable $ 55
- Circular/flat ride less than 20 ft in height $ 75
- Spectacular ride $100
- Coasters over 30 ft in height $200
- Coasters over 60 ft in height $400
- Concession Go-Karts
- Up to 20 Go-Karts $300/Track
- Over 20 Go-Karts $ 10/Go-Kart
- Zip Line $150/Line
- Large generator $165
The total permit fee owed at the time of application is the permit fee plus the 2% state levy.
A 75% discount on the permit fee is applied when using a private inspector.
A 50% surcharge will be added to the permit fee if an inspection is requested to be performed on a weekend.
How to Receive this Service
Online application is not available for amusement devices. Please mail or hand deliver the following documents along with the permit fee to the Department of Building Construction and Inspections:

1 copy of insurance certificate
1 copy of current inspection certificate (kiddie rides only)
1 copy of Owner’s Statement of Responsibility
Permit Cancelation
1 copy of Request for Permit Cancelation
Note: When cancelling a permit, the refund is 100% if the permit has not been issued. If the permit has been issued, the refund is 75% of the permit fee.
If a refund is requested, a reason for the cancellation must be added to the cancellation form.
