The Richmond City and Henrico County 2025-2028 Community Health Improvement Plan (CHIP) is a
roadmap for improving the health and wellbeing of our community. This plan was developed from the
findings of the 2024 Richmond City and Henrico County Community Health Assessment (CHA)
