Forensic Discharge Program

The Forensic Discharge Program is a voluntary post sentencing mental health program. Mental Health case management services are offered to individuals diagnosed with a serious mental illness who are preparing for their release after being sentenced. A case manager begins work with the individual while they are incarcerated and develops a release plan conjointly with the participant that includes linkage to entitlements, resources, medication management, clinical services, psychiatric services, case coordination and case management services. The case manager continues to work with the participant for 30-90 days post release and transitions the participant to long term services in the community.
